The following documents shall be submitted to CAAC

1.        Application for issue of maintenance organization certificate (F145-1).(Sample).

2.        Revised Maintenance Capability List in which the new added components shall be clearly indicated (only applicable for applicant applying for new components other than complete powerplant or propeller).(Sample).

3.        Maintenance Capability list in Excel (If item 2 is applicable).(Sample).

4.        Letter of Intent from Chinese customer with all the new ratings and/or components applied for CAAC approval with both stamp and signature.

5.        A copy of current & effective approval for the new added rating and/or components issued by the local/regional civil aviation authority, or a statement to indicate that the new added rating and/or components are approved or accepted by the local authority.

6.        Internal evaluation report(s) with signature of Quality Manager for those parts being added to show compliance with regulation.

7.        A general introduction of the Maintenance documents and equipment used during repair for the new added rating or components (Especially for DER and/or DOA repair, self-made equipment).

8.        General introduction of subcontract functions for the maintenance of new rating or components.

9.        Revised Maintenance Management Manual (If applicable).

Conclusion of Approval of revised maintenance capability.

CAAC will re-audit the added components during the next on-site audit.
